Select Create form and pick a template instead of a blank form. Browse by topic or search by name, then choose Use this template. It arrives as a draft for you to change.
Your own templates
Choose Save as template from a form's menu on the dashboard, or from the form's header. Your templates appear under Your templates when you create a form.
Sharing with your team (Business)
On Business, choose Share with the team on one of your templates and everyone on your team can start from it. Choose Stop sharing with the team to take it back.
