Add a File upload field and respondents get a drop zone they can drag onto or select to browse.
Size
Each file can be up to 20 MB on Free, 150 MB on Pro and 250 MB on Business. The plan of the form's company decides. The limit is shown to the respondent under the drop zone, so nobody wastes time on a file that will be refused.
Accepted types
Leave the accepted list blank to take anything. Otherwise, list extensions such as .pdf .png .docx and the picker filters to those.
Where files go
Uploaded files are attached to the response. Open the response in the inbox to download them. In a CSV or Excel export, each file is a link in its question's column rather than the file itself.
